6. How to File a Grievance

If you believe that Complete Health Wellness Group LLC has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with us.

Grievance Process

  • You can file a grievance in person, by mail, by phone, or by email
  • We will acknowledge receipt of your grievance within 5 business days
  • We will investigate and respond to grievances promptly, typically within 30 days
  • You may file a grievance anonymously, though this may limit our ability to investigate or respond

Important: Filing a grievance will not affect your care, treatment, or ability to receive services from our practice.


7. Filing a Complaint with the U.S. Department of Health and Human Services

You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, if you believe you have been discriminated against.

Time Limit: Complaints must generally be filed within 180 days of the date you knew or should have known about the alleged discrimination. The Office for Civil Rights may extend this timeframe for good cause.


8. No Retaliation

We will not retaliate against any person who files a complaint, testifies, assists, or participates in any manner in an investigation or proceeding related to discrimination.

You may file a grievance or complaint without fear that it will affect your care, your relationship with your therapist, or your ability to receive services from our practice.

Retaliation against individuals who assert their civil rights is prohibited by federal law. If you believe you have experienced retaliation, you may file a separate complaint with the Office for Civil Rights.


9. Additional Maryland State Protections

In addition to federal protections, Maryland state law provides additional protections against discrimination. The Maryland Commission on Civil Rights enforces state civil rights laws.

Maryland law prohibits discrimination in places of public accommodation on the basis of race, color, religion, sex, age, national origin, marital status, sexual orientation, gender identity, and disability.
