New Client Services Process

1) Initial Contact

  • Submit the Consultation Request Form (button above) or contact our office directly.

  • Our client care team will follow up within one business day or less to review and provide additional information.

 

2) Confirmation Call

  • Speak with our client care coordinator to confirm information and the provider's availability.

 

3) Phone Consultation

  • Schedule and complete a phone consultation with the provider.

 

4) New Client Forms

  • Complete all new client forms within 3 days of the phone consultation.

  • An email will be sent with a link to access your portal account for electronic form completion.

  • All forms must be completed at least 2 business days before the first appointment date.

 

5) Insurance Verification

  • Upload a copy of your health insurance card to verify benefits prior to the first appointment.

 

Please note that the availability of our providers depends on multiple factors; including type/complexity of concern, scheduling availability of the client and preferred provider, health insurance participation, and geographical location.

For general questions or requests, please call our office. A member of our team will promptly follow up to inquiries received outside of our office hours.

  • How to schedule as a new client?
    Please contact our office by calling, emailing, or submitting a Consultation Request Form.
  • Are teletherapy and office appointments available?
    We are offering both in-person and teletherapy appointments. Location availability will depend on the therapist.
  • Are the therapists accepting new clients?
    Yes. Please contact our office to speak with one of our client care coordinators to check the availability of a specific therapist.
  • Does the therapist prescribe medications?
    No. Our practice does not currently have a provider licensed to prescribe medications.
  • Does the therapist accept my health insurance?
    Our practice accepts several health insurance plans from different insurance companies. We are happy to assist with reviewing your health insurance to help determine if our practice and therapist accept your plan. Our Insurance page provides additional information. Please call our office to speak with our client care coordinator for additional questions.
