Maryland Privacy Notice

Effective Date: 2025-10-01

Last Updated: 2025-06-15

Version: 1-20250615

This Maryland Privacy Rights Notice supplements the information contained in our General Privacy Policy and applies solely to residents of the State of Maryland (“consumers” or “you”). Complete Health Wellness Group LLC has adopted this notice to comply with the Maryland Online Data Privacy Act (MODPA).

1. Our Commitment to Your Privacy

Under MODPA, Complete Health Wellness Group LLC is considered a "data controller." We are committed to protecting your personal data and handling it responsibly. This notice explains the specific rights you have under Maryland law and how we approach the collection and use of your information, particularly sensitive information, before you become an established patient.

2. Key Definitions

  • Personal Data: This is any information that is linked or can be reasonably linked to you. This includes basic identifiers like your name and email address, as well as online identifiers like your IP address.

  • Sensitive Data: This is a special category of Personal Data that receives heightened protection. For our services, the most important type of sensitive data is Consumer Health Data, which is any personal data we use to identify a consumer’s physical or mental health status. Information you might consider providing about your reasons for seeking therapy is considered Consumer Health Data.

3. Data Collection: Our "Strictly Necessary" Standard

MODPA sets a high standard for privacy, especially for Sensitive Data. We are required to limit our collection of your Personal Data to what is "reasonably necessary" for the service you request. For Sensitive Data, including Consumer Health Data, the standard is even higher: we may only collect it if it is "strictly necessary" for the service you have requested.

  • What this means for you: When you first contact us through our website's "Consultation Request Form," the service you are requesting is to schedule a consultation. To meet the "strictly necessary" standard, we will only collect the minimum information needed to fulfill that specific request, such as your name and contact information. We have intentionally removed open-ended questions about your health from our initial contact forms to protect your privacy and comply with the law. The collection of more detailed health information will be deferred to a later, more secure stage of the intake process (for example, through a secure patient portal) after you have decided to proceed.

4. Your Rights Under MODPA

As a Maryland resident, you have the following rights regarding your Personal Data:

  • Right to Access and Confirm: You have the right to confirm whether we are processing your Personal Data and to access that data.

  • Right to Correct: You have the right to correct inaccuracies in your Personal Data.

  • Right to Delete: You have the right to request the deletion of Personal Data that you have provided to us or that we have obtained about you.

  • Right to Data Portability: You have the right to obtain a copy of your Personal Data in a portable and, to the extent technically feasible, readily usable format.

  • Right to Opt-Out: You have the right to opt out of the processing of your Personal Data for the purposes of:

    • Targeted advertising;

    • The sale of personal data; or

    • Profiling in furtherance of decisions that produce legal or similarly significant effects concerning you.

Complete Health Wellness Group LLC does not sell your Personal Data. We especially do not sell Sensitive Data, which is strictly prohibited under Maryland law.

5. How to Exercise Your Rights

To exercise your rights, please see the "Contact Us" section below. We will respond to your request within 45 days of receipt. We may extend this period by an additional 45 days when reasonably necessary, and we will inform you of any such extension within the initial 45-day period. If we decline to take action on your request, we will inform you of the reason and provide instructions on how you can appeal our decision. You have the right to contact the Maryland Attorney General if you are not satisfied with the outcome of your appeal.
